Media Indexing all the time

I am constantly getting notices that my system is sluggish and when I check its due to media indexing. Nothing is changing on any location that should be indexed. I have an FTP upload to a folder but that folder system is removed from indexing so I can only assume that removing an item from indexing isn't working currently.
Is anyone else seeing this?

Another clue this may be the case is when accessing Photos on the TNAS Mobile 3 app I can see photos from the folder that shouldn't be being indexed.

Which TOS version are you using? Please update it to the latest.
If the indexed data is incorrect, you can rebuild the index.

Running TOS 5.0.176-00227 and system shows its the current version.
How do you rebuild the index? I only see options to turn it on and off.
