Can you confirm how this should work on an iPhone

I take photos on my phone, do I then have to load the app, login and then tap it to transfer the photos?

I.e it’s still a manual process, it can’t just upload in the background as I take photos, I still have to remember to sync them?

No, it doesn’t run backend always, you need to activate it.
