Please check if the USB drive is healthy and try to clean the partition of USB with your computer and try again.

Please can you clarify what you mean by "clean the partition"? Remove all partitions? Create an empty partition but don't format it?

Can anyone else step in here and describe EXACTLY how the USB drive should be prepared?

You can use a tool like RMPREPUSB to clean the drive.

Then let Windows format it afterwards, to create a single FAT32 partition.

I think I have now successfully created a working boot drive, but I still received warnings during the process. This is the output:

Same issue: Installing for x86_64-efi platform. grub-install: error: /media/ doesn't look like an EFI partition How do I clean a blank partition? Just delete all partitions on the USB drive? Some sites say create a FAT32 partition. That did not work as well. PLEASE TYPE BASIC LAYMAN TYPE ANSWERS. BETTER YET, POST A DOWNLOADABLE IMAGE. THANK YOU.
