Raid Manager Pro will not connect

I have installed 4 new 6TB drives and the D5 will not connect. It is not asking for a username or password, I just continually get the following error:
The connection between APP and RAID HBA has been interruptted, APP will try to reconnect RAID HBA in ___ second(s).
How can I reset the RAID Manager so I can access the username and password field?
