I'm running an F4-220 on TOS 4.1.118 with 4x HGST 7200RPM 4TB NAS HDD. They're configured in RAID5.
Woke up to my NAS beeping at 3AM. Logging in shows an exception from storage pool 1, "HDD missing." Sure enough, when you check the storage pool in the control panel, HDD #1 is not listed. The other 3 drives show "Good" status.
However, when you look at the "Hard Drive" option in the control panel, HDD #1 is listed and and screen says there are 4x "Good" drives. Defective and At Risk are zero.
I'm a little confused as to how to proceed. Do I actually have a dead HDD? Is there some other procedure I need to carry out?

The hard disk appears to be dropped. There are many reasons for this.
You can search the Internet for the reasons that professionals have said.
You can try to re-install HDD1 into the device for synchronization.
If it cannot be synchronized, you can format it on your computer and then synchronize again.

Because of some issues happened to your drive, it dropped off from your RAID 5.
The S.M.A.R.T tools list in the TOS UI is not powerful enough to determind if your drive is really healthy. I have the same issue. I ran to check for bad sectors and it said no bad sectors
You mentioned to format it. Do I use windows 10 and remove the existing partitions and format it completely

4.1.18 is quite old version, why not update it to latest one?
