In particular, I am wondering how the "Custom" permissions work... is Host/Domain looking for a hostname, an IP address, etc? In my experience, putting a valid domain name in the field effectively prevents the initiators from connecting.
This is under 5.1.29.

Try setting it to use "All network interfaces", rather than just LAN1. I have had similar issues under TOS5.1, where enabling just one NIC in the target settings results in no access to ISCSI.

Looks like this was my bad. I read the release notes for 5.1.29, but did not check the release announcement here in the forums.
